Registration Forms

You need to:

1) register with the HKHA by filling out and submitting

men this HKHA form or ladies this HKHA form

2 photos signed on the back

a copy of your HK-ID card or passport copy including the HK entry stamp page in the case that you have no HK-ID card;

put all itmes into one envelope.

Put the envelope into the Hockey Section pigeon hole in the Club foyer addressed to the appropriate HKHA convenor namely Stephen Bottomley* for the men and 'HKHA-WS Registration' for the female players. 

2) submit a HKFC Club registration form (click here for form) and one photo in an envelop addressed to the appropriate HKFC Membership Secretary, namely Nick Studholme-Wilson for the men and 'Ladies HKFC Hockey membership - Kath Sly' for the female players.

*For the men contacting Stephen if you put a form in the hockey pigeon hole then please email Stephen to tell him you have done that. Alternatively you may send/deliver the form to Stephen's office address below (first preference):
