Registration Forms

To play in the Hong Kong Hockey Association's leagues as a member of the HKFC Hockey Section involves the completion of two separate sets of paperwork. A summary is set out below.
A.            CLUB MEMBERSHIP WITH HONG KONG FOOTBALL CLUB
1.             The initial stage in HKFC Membership is to fill in a Registered Temporary Player (RTP) form on your first attendance at the Club. Once completed, please place the form in the Hockey Section pigeon hole in the Club foyer address to the appropriate Membership Secretary: Andy Dale for the Men’s Section or Jocelyn Teh for the Women's Section. Once signed off by Andy or Jocelyn, the RTP form will be submitted to HKFC’s Membership Department.
2.             The RTP form entitles you to attend training or matches and gives you access to the changing rooms for up to 30 days. During this period, coaches and captains will decide if Hockey Section supports your membership. If we do support your membership, the Membership Department will arrange an appointment with you to complete a Sports Preferred Membership Application and explain the terms and conditions of Membership.
3.             If approved, you move on to Temporary Sports Preferred Membership, which gives you a Club Membership Card and use of all facilities for 5 months, before you are interviewed by the Membership Sub-Committee. If successful, you will be transferred to a Full Sports Preferred Membership of the Club. 
B.            REGISTRATION WITH HONG KONG HOCKEY ASSOCIATION MEN'S SECTION and WOMEN'S SECTION
The Men's Section ("HKHAMS") and the Women's Section ("HKHAWS") are administered as two different sections hence the split below. All communications with these Sections are made through the Men's Convenor (Stephen Bottomley) and the Women's Convenor (Rhona Barr).
HKHAMS
1.             You will be registered by the Men's Convenor with the HKHAMS once:
·               you have filled out Sections 1, 2 and (where appropriate) 3 of the registration form (click here for HKHAMS form)
·               you provide a photocopy of your HK ID card with your photo showing clearly
·               you deliver the form and photocopy into one envelope.
2.             You will probably already be in touch with the Men's Convenor but, to avoid disappointment and delay, you should email the Men's Convenor (stephen.bottomley@mayerbrownjsm.com) to alert him that your registration form is about to be delivered.
3.             You can put the envelope into the Hockey Section pigeon hole in the Club foyer addressed to the Men's Convenor.
4.             Alternatively, you can deliver it to Stephen Bottomley at Mayer Brown JSM, 18th Floor, Princes' Building, 10 Chater Road, Central.
5.             The application to HKHAMS usually takes about 5 days to clear. If you ensure that the papers are delivered to the Men's Convenor by 3.00pm on a Monday at the very latest, the Men's Convenor can usually get you registered to play on the Sunday.
HKHAWS
1.             You will be registered by the Women's Convenor with the HKHAWS once:
·               you have filled out Sections 1, 2 and (where appropriate) 3 of the registration form (click here for HKHAWS form)
·               you provide a photocopy of your HK ID card with your photo showing clearly; a scanned copy sent by email to the Women’s Convenor (rhonabarr@yahoo.com) is also acceptable
·               A recent passport photograph; a scanned copy of this may also be sent by email
·               you deliver the form, photograph and photocopy into one envelope.
2.             You will probably already be in touch with the Women's Convenor but, to avoid disappointment and delay, you should email the Women's Convenor (rhonabarr@yahoo.com) to alert her that your registration form is about to be delivered.
3.             You can put the envelope into the Hockey Section pigeon hole in the Club foyer addressed to the Women's Convenor.
4.             Alternatively, you can deliver it to Rhona Barr at Kellett School, 2 Wah Lok Path, Wah Fu, Pokfulam.
5.             The application to HKHAWS usually takes about 5 days to clear. If you ensure that the papers are delivered to the Women's Convenor by 3.00pm on a Monday at the very latest, the Women's Convenor can usually get you registered to play on the Saturday.

You can only play for the Club when the membership process of the Hong Kong Football Club and registration with HKHAMS/HKHAWS have both been completed.
